Terms and Conditions I understand by filling out and submitting this form, my child is officially enrolled in class(es) at Studio Jacked dance co for a ten-month period (Sept 3, 2025- June 18, 2026) I also understand that tuition payments are collected through our automated system and that the credit card that we have on file will be charged on the 1st of every month. I understand that New students will be charged at $35 registration fee upon registering. I understand that I will be charged for the entire month upon registering. (If you register late in the month, you are welcomed to take other classes in your age range to make up the amount of classes you missed.) I understand that if my credit card is declined, Studio Jacked will automatically try to run it again. If it is declined a 2nd time, I will be informed and will be able to provide an additional method of payment at that time. After the 2nd attempt of collecting declined payments, a $15 late fee will be added. Students with outstanding balances will not be permitted to dance. No excuses. In the case that I need to withdraw from dance classes, I agree to inform Studio Jacked IN WRITING and will be responsible for my tuition payments and any outstanding balances until written consent is received. (If you withdraw before the month is over, no refunds are given. You will be allowed to finish up the month or make up the classes in any class that is your age range.) I am aware that all Studio Jacked Dance Company classes & instructors are subject to change. GENERAL LIABILITY WAIVER I understand and agree that participating in any dance lesson or class, workshop, rehearsal, or performance, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me (or my child) during any of Studio Jacked Dance Company classes, lessons, rehearsals, performances, or activities. I also exempt, release, and indemnify Studio Jacked Dance Company, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me or property which may arise out of or in connection with participation in any classes or activities conducted by Studio Jacked Dance Company. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Studio Jacked Dance Company, its owners, employees, volunteers, assistants, guest artists, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. By signing this, Parents/legal guardians give their permission to Studio Jacked Dance Company to use photos and or video of their child without remuneration in connection with studio publications, social media, advertising, tv, and news coverage. ASSUMPTION OF RISKS AND WAIVER OF LIABILITY RELATED TO COVID-19 Studio Jacked Dance Company has put in place preventative measures to reduce the spread of COVID-19; however, Studio Jacked cannot guarantee that you will not become infected with COVID-19. Further, attending classes at Studio Jacked could increase your risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending classes at Studio Jacked Dance Company and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Studio Jacked Dance Company may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Studio Jacked Dance Company employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim liability or expense of any kind that I may experience or incur in connection with my participation to Studio Jacked Dance Company classes (“Claims”). I hereby release, covenant not to sue, discharge, and hold harmless Studio Jacked Dance Company, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Studio Jacked Dance Company, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any class at Studio Jacked Dance Company. Covid part 2: I understand and agree to all the new COVID-19 related rules and protocols administered by Studio Jacked Dance Company. I understand if my child is sick they will not come to class and must have a doctor’s note clearing them from COVID-19 before they can participate, or stay out of dance for at least 14 days symptom-free, whichever comes first. I understand my child will stay home from dance if someone in my immediate household contracts the virus, and must have a doctor’s note clearing them from COVID-19 before they can participate, or stay out of dance for at least 14 days symptom-free, whichever comes first. I understand I am willingly and voluntarily allowing my child to participate in an activity where they are around other people, thus increasing their risk of contraction. I understand the risk associated with allowing my child to participate in dance classes regarding COVID-19. However, while I also understand Studio Jacked Dance Company will do whatever it can to prevent the spread of the disease, Studio Jacked Dance Company cannot be held responsible should any child, parent, or guardian contract COVID-19 while utilizing Studio Jacked Dance Company services and premises. I hereby release and discharge Studio Jacked Dance Company and any of its owners, directors, officers, and employees from any liability whatsoever which may arise as a result of the student participating in events sponsored by Studio Jacked Dance Company
